Background: Limited options exist for young patients with massive, irreparable rotator cuff tears. While several treatment exist, superior capsular reconstruction (SCR) was developed to restore glenohumeral joint stability by preventing humeral head migration during motion. Indications: Arthroscopic SCR is indicated in a tear. Patients require functioning deltoid muscle minimal no arthritis. In addition, should have Hamada grade 2 or less and an intact repairable subscapularis tendon. Technique Description: The patient positioned the beach chair position. Standard anterior, posterior, mid-lateral portals are established. An accessory posterior-superior lateral portal used visualization. Three 3.0 mm knotless anchors placed glenoid through percutaneous incisions. Two 4.75 adjacent articular cartilage medial row fixation. graft dimensions measured using distance between anchors. prepared gown card as template 5 added medial, anterior dimensions, 10 dimension. Sutures passed inverted horizontal mattress diminish friction allow appropriate suture passage mechanism of anchor. then shuttled into PassPort button cannula. secure on humerus. Native tissue repaired sutures from well free SutureTape. Results: Outcomes following demonstrated improvements clinical outcomes shoulder range Similar been identified use allograft autograft. generally able return light recreational activities work.
